The Levels of Serum Estradiol, Urine Deoxypyridinoline and Maturation Index in Korean Postmenopausal Women Treated with Estriol Vaginal Tablet

Abstract
|
|
|
Objectives: Our purpose was to assess systemic effects and local effects on the vaginal mucosa of estriol vaginal tablet by measuring the levels of serum estradiol, urine deoxypyridinoline, and maturation index after estriol vaginal tablet treatment aimed for treatment of vaginal atrophy.
Methods: In treatment group, thirty healthy postmenopausal women were recruited to treatment with 500?g estriol vaginal tablet for 12 weeks. Finally, 21 women were remained in treatment group. We compared the levels of serum estradiol, urine deoxypyridinoline, and vaginal maturation index before treatment with those after estriol vaginal tablet treatment for 12 weeks. In placebo group, we compared the levels of urine deoxypyridinoline before placebo insertion with those after 12 weeks. Values were compared using Wilcoxon signed rank test.
Results: In treatment group, serum estradiol (E2) concentration decreased from 15.1±3.7 (SD) pg/ml to 12.9±2.7 (SD) pg/ml and urine deoxypyridinoline (uDPD) concentrations decreased from 8.5±3.8 (SD) nM DPD/mM cr to 6.6±2.9 (SD) nM DPD/mM cr after estriol vaginal tablet treatment for 12 week (E2 P=0.074, uDPD P=0.003, respectively). In placebo group, urine deoxypyridinoline (uDPD) concentrations also decreased from 9.9±8.3 (SD) nM DPD/mM cr to 5.2±1.8 (SD) nM DPD/mM cr after 12 weeks (P=0.028). The mean maturation index has significantly shifted from 82：15.5：2.5 to 3：86.5：10.5 (% parabasal cell: intermediate cell: superficial cell)(P=0.005：0.007：0.032, respectively).
Conclusion: Twice weekly 500 ?g estriol vaginal tablet treatment for 12 weeks has local effects on vaginal mucosa without systemic effects in postmenopausal women. It can be used safely for 12 weeks.
